Elitist Healthcare medical evaluations and treatments are all under the care of our Board-Certified Family Medicine Doctor and Medical Director, Dr. Carlyn Sainvil.

• one (1) - 60 minute initial visit
• one (1) - follow up visit for treatment, medications, and lab review within 30 days of first visit

• After New Patient Consultation, one (1) routine visit per month
• $75 each additional visit 
• Priority scheduling
• No charge for prior authorizations review and completion
• 20% discount on form review and completion (FMLA, disability, medical)
• Easy access and and open communication via phone, patient portal, and emails 

• After New Patient Consultation, each follow up visit is a charge of $200
• Fee for all additional visits with no membership
  • Prior Authorization - $25 per prior authorization
  • Forms (such as FMLA or disability paper work) - $150
